If you want your medical records to be sent to our office from another office, please print the Medical Records Release form, complete it, and fax, deliver or mail it to the office which currently has your medical records.
Disability forms and other letters
There is a $15 fee for completion of forms and/or the production of letters during a patient's care. This would be for purposes other than forms used to submit charges for services rendered.
