How can I pay for my visit? We collect your co-pay at the time of check-in, so please be prepared to pay this prior to seeing your provider. We accept cash, checks, money orders, Mastercard or VISA. The remainer of your visit cost will be billed directly to your insurance carrier. Please be sure to let our office know if there have been changes in your coverage. We are currently contracted with the insurance companies listed here. Please let us know if you have a question about your specific plan. If you do not have insurance coverage, you will need to cover the total cost of your visit on the day of service. There may be additional costs for labs and other tests that will be billed directly to you. There is a $20 fee for returned checks. Our fees and methods of payment are comparable to other OB/GYNs in the area. If we participate with your insurance, we will file your insurance for you and ask you to pay any deductible or co-payment amounts. If we do not participate with your insurance, we will ask you to pay in full on the day of service. We will provide you with an itemized receipt which you may choose to submit to your insurance company. Insurance company list If you are scheduled for surgery or are an obstetrical patient, we will contact your insurance company as soon as possible to learn your benefits. Although insurance companies do not guarantee benefits, we will estimate your portion of the surgical or obstetrical fees and ask that you pay this amount prior to your surgery or delivery. There is a $15 fee for completion of forms and/or the production of letters during a patient's care. This would be for purposes other than forms used to submit charges for services rendered. This fee is due from the patient prior to the release of the form/letter. These forms include, but are not limited to:
